OK, some housekeeping... important forms for you teams to fill out.

- Completing a team info doc to help officials remember your team
- Consent forms

Team Info Sheets:
This electronic PDF form is used to capture team profile information; it requires Adobe Acrobat Reader (available for free at www.adobe.com). Please make your best effort to send this back to me before the tournament. I'm hoping to be able to provide each team’s completed version to all judges and the MC, so they can have some background info on each team!

Consent forms:
Consent forms must be completed by all those involved with teams not just the students. So mentors, coaches volunteers must compete a form and turn it in at the November Tournament.
I cannot guarantee that we will have enough printed copies on hand for those who have forgotten to print and bring theirs so please do you best to complete the forms in advance and bring them to the November 28th, 2009 tournament for collection.
